Volume 4, Issue 22 November 1, 2000 Key InformationNew Procedure For Prescription Safety GlassesSLAC will reimburse up to $125.00 toward the cost of single vision prescription safety glasses for employees who need prescription lenses in jobs that require eye protection . The employee must complete a Prescription Safety Glasses Approval Form to be reimbursed. The form must be signed by a manager or supervisor before the prescription safety glasses are purchased. The form is available at the Petty Cash Office or Human Resources, or on-line at the direct URL: http://www-bis.slac.stanford.edu/forms/safety_glasses.html (From the BIS HR Forms page: click on: Prescription Safety Glasses Approval Form). The reimbursement cost will be charged to the employees department/group. Department heads, supervisors, and managers decide who within their department/group require prescription safety glasses. Additionally, the Safety, Health & Assurance (SHA) Department and SLACs Medical Department are available to assist in defining eye-hazard operations. Generally, Occupational Safety and Health Administration (OSHA) regulations require eye protection safety goggles or spectacles -- where there is a reasonable probability of injury that can be prevented by such equipment. Employees may select an Optician of their choice (this is a change from the prior procedure, which required employees to use a SLAC-designated vendor). However, only qualified optical personnel should fit prescription safety glasses. The following Opticians currently offer Stanford/SLAC employees a discount on their prescription glasses:
All prescription safety glasses purchased after July 5, 1994, must comply with ANSI Z87.1-1989, the American National Standard Practice for Occupational and Educational Eye and Face Protection Standard. The Standard sets forth requirements for the design, construction, testing, and use of eye protection devices. (For example, there can be no insertion of non-safety lenses in safety frames or vice versa.)
