If you are claiming Tax Treaty you must complete form 8233 for each tax year.

Please submit to Payroll a completed 8233 Form 2006 on or before December 21, if possible.

Forms are available in the Records Department in Human Resources (A&E Bldg. 41, Rm. 236). If you fail to submit the form, federal tax will be deducted from your salary until the completed form is received.

You will also need to fill out a new W-4 for 2006. This form is available in the Payroll Office or on-line at: http://www-group.slac.stanford.edu/travel/PAYROLL/W-4
